Copies of Birth Certificates
WHO MAY OBTAIN A BIRTH CERTIFICATE?
If you are 18 years of age or older, you can obtain a certified copy of your own birth certificate. A parent can obtain the birth certificate of their child, if the parent’s name appears on the child’s birth certificate.
HOW TO OBTAIN A BIRTH CERTIFICATE?
IN-PERSON:
Come into the Washington County Clerk’s Office, 101 E. St. Louis St., Nashville, IL 62263, Monday-Friday, 8:00 AM to 4:00 PM and fill out an application.
BY MAIL:

Fill out the application and mail it along with a photocopy of a current and valid acceptable form of identification and a check or money order made payable to the Washington County Clerk to:

WHAT ARE THE FEES FOR A BIRTH CERTIFICATE?
The fee is $15 for the first copy and $5 for each additional copy of the same certificate, requested at the same time.
